Optionally to create foreign key constraint without creating index on it. In some cases this is a duplicate unusable indexes. On big tables that impact performance.

This is a non-trivial change because foreign key checking is done in the
index code. It would be easier to recognize that an existing index could
be used than to allow a constraint without any index at all.
